Up to two supporting documents that are directly relevant (for example, your decision letter, assessment report, or previous award notice).
What Not to Send
This service is intended to review the document you have prepared, not your entire case file.
Please do not send:
Full GP medical records.
Large bundles of hospital letters.
Extensive collections of medical evidence.
Multiple years of supporting documents.
If, after reviewing your document, we feel that additional evidence would strengthen your case, we'll tell you what type of evidence may be helpful and why.

Where an electronic version is available, we'll send you a copy that you can complete on your computer or device. This makes it much easier to type your answers, save your progress, make changes and email the completed document to us for review.
